Growing together. You will be part of the primary care teamcollaborating with all interdisciplinary team members and willsupport multiple providers. The Patient Care Coordinator is aflexible team member who works to support nurse practitioners (NPs)and physician assistants (PAs) as well as our patients. This roleassists in care coordination activities, administrative tasks andsupports our patients and providers. This position is full time,Monday
- Friday. Employees are required to have flexibility to workany of our 8-hour shift schedules during our normal business hoursof 8:00am
- 5:00pm.
It may be necessary, given the business need,to work occasional overtime. We offer 2 months of paid training.
The hours of the training will be based on schedule or will bediscussed on your first day of employment.
PrimaryResponsibilities:
- Support interdisciplinary care team with overall member coordination at the direction of RN or NP/PA
- Dataentry and member chart management for: New members, post hospitalvisits, quality, labs and medications
- Obtain medical recordsfollowing hospitalizations or physician appointments

RequiredQualifications:
- High school Diploma / GED OR equivalent workexperience
- Must be 18 years of age OR older
- 3 years of clinicalor medical record experience. Strong knowledge of medicalterminology and EHR platforms
- Access to reliable transportationthat will enable you to travel to patient locations
- Ability totravel up to 100% of the time
- Must reside in Albany, NY
- Abilityto work full time, Monday
- Friday. Employees are required to haveflexibility to work any of our 8-hour shift schedules during ournormal business hours of 8:00am

PreferredQualifications:
- Certified nursing assistant or medical assistantcertifications
- Ability to keep all company sensitive documentssecure (if applicable)
- Required to have a dedicated work areaestablished that is separated from other living areas and providesinformation privacy
- Must live in a location that can receive aUnitedHealth Group approved high-speed internet connection orleverage an existing high-speed internet service The hourly rangefor this role is $19.
86 to $38.85 per hour based on full-timeemployment. Pay is based on several factors including but notlimited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
