JOB SUMMARY:
The Health and Disabilities Coordinator (HDC) holds a critical position entailing the comprehensive management and supervision of all facets of health and disabilities services within the program. Responsibilities include assuring the delivery of superior health, oral health, mental health, and nutrition services that are of exceptional quality and developmentally, culturally, and linguistically suitable, thereby supporting each child’s developmental progress and school readiness. Moreover, the HDC is instrumental in disabilities services, undertaking active recruitment of children with disabilities to fulfill the mandate of allocating 10% of the program’s funded enrollment to this demographic. This involves close collaboration with all pertinent stakeholders, including parents and legal guardians, to secure the timely and suitable provision of services to children with suspected or diagnosed disabilities. The HDC is further tasked with guaranteeing that all children have complete participation and access to the full spectrum of program activities and services. In fulfilling these responsibilities, the HDC ensures adherence to all relevant federal, state, and local regulations, including the Head Start Program Performance Standards, the Head Start Act, and regulations set forth by the NYC Department of Health and NYC Department of Education. Additionally, the HDC ensures compliance with all BSECDC policies and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The HDC’s duties and responsibilities are multifaceted and extensive. Collaborate with the HS Management Team to formulate policies, procedures, and written plans for health and disabilities services. Monitor compliance with these work plans, Head Start Program Performance Standards, and other regulations. Additionally, develop and implement an ongoing monitoring plan for continuous improvement and evaluation. Primary point of contact for health services contracted providers, such as those for nutrition, nursing, and mental health services. As well as the contact for local education agencies (LEA) and early intervention providers to coordinate the multidisciplinary evaluation, determination, and provision of services to children with suspected and identified disabilities. Communicate between contracted health service providers, program staff, and parents. They are responsible for establishing and maintaining a Health Services Advisory Committee that includes Head Start parents, professionals, and other volunteers from the community. Regular monitoring and analysis of health and disabilities data is another key aspect of the HDC’s role. This data is used to develop plans that respond to identified needs and ensure compliance with all state and local requirements. The HDC also ensures proper documentation and completion of health, nutrition, and safety screenings, exams, and follow-up, ensuring that all documentation is complete and accurate. Guarantee the quality of services, the HDC develops and implements an ongoing monitoring system. This includes regular site visits, file reviews, analysis of data, staff interviews, and observations. The goal is to ensure that the needs of children with health-related and special dietary needs, as well as those with disabilities, are met, with necessary adjustments made along the way. Collaboration with the Education Site Managers (ESM) is crucial to ensure that all required child screenings and assessments are completed promptly. The HDC reviews results and coordinates follow-up for children with identified needs. Providing leadership and support to the ESMs and Family Assistants to ensure that all health and disabilities services activities meet the related requirements and result in high-quality services for children and families. Ensure that children receive the appropriate and required health services and disabilities services, if applicable. They monitor health and disabilities services delivery and use the results to develop plans that support progress towards program goals and ensure compliance. Additionally, they support ESMs and Family Assistants by providing resources and establishing community partnerships to ensure that children’s health status is up-to-date, that children receive ongoing or follow-up care as needed, and that parents have the information needed to support their children’s overall health and wellness. Participates in meetings with parents of children who are suspected of having a disability to discuss the need for further evaluation and the benefits of early intervention to support children’s academic achievement. They also participate in IEP and mental health consultation case management meetings, as appropriate. Assists parents in developing their advocacy skills by engaging them in obtaining timely services for their children. They work closely with families to ensure that service delivery is culturally appropriate. They also coordinate services with the mental health consultants based on teachers’ observations and/or parent input to support children who need social and emotional support, while coordinating with the family service staff to work with their parents to address family well-being issues. Coordinates the provision of any necessary modifications to the environment, multiple and varied formats for instruction, and individualized accommodations and supports necessary to support the full participation of children with disabilities. Assist teaching staff, as needed, to identify home-school activities that support children with suspected and identified disabilities and/or are designed to improve social-emotional development. Assists parents of children with disabilities with transitions within and out of the program by engaging them in the LEA’s review meetings, planning the transfer of relevant documents, and selecting the appropriate learning environment. Participates in the annual review of the Community Assessment or Community Assessment Update and, in consultation with staff, parents, and other stakeholders, develops recommendations for revisions to recruitment strategies for children with disabilities. Analyze health and disabilities data to inform decision-making for service area plans, community partnerships, and training and professional development for Family Assistants, ESMs, and other staff, if needed. Provide health and disabilities services status reports to the Management Team, Executive Director, Board, and Policy Council. Establishes and monitors agreements with community partners to support the recruitment of all children and those with disabilities. Ensures that all individuals with disabilities enrolled in the program are protected from discrimination under and provided with all services and program modifications required by Section 504 of the Rehabilitation Act (29 U.S.C. 794), the Americans with Disabilities Act (42 U.S.C. 12101 et seq.), and their implementing regulations. ASQ MINIMUM QUALIFICATIONS/EXPERIENCE
Bachelor’s Degree in a related field preferred Three to five years of experience working with children and families with special needs.

PRE-EMPLOYMENT REQUIREMENTS
Submission of three (3) professional references. Completion of mandated reporter training and acquisition of certification (renewal required every two years), along with adherence to other training requirements stipulated by the Department of Health and Mental Hygiene (DOHMH) and Head Start Performance Standards (HSPS).
