Role:
As a Patient Coordinator , you’ll be the vital link between patients, families, caregivers, and our clinical team. Your role is to help coordinate services, ensure smooth communication and scheduling—so that our patients receive timely, compassionate, and effective home care.
Key Responsibilities:
Serve as the main point of contact for patients and families Coordinate caregiver assignments and maintain accurate scheduling Collaborate with nurses, case managers, and other team members to support care plans Track patient services and ensure documentation is complete and compliant Respond to patient inquiries with empathy and professionalism Complete daily/weekly task reports Must be able to manage a caseload of 100 HHA/PCA cases
Qualifications:
Previous experience in home care, healthcare coordination, or a related administrative role Excellent communication and customer service skills Ability to multitask and stay organized in a fast-paced environment Familiarity with HHA exchange or scheduling software a plus Bilingual skills are a strong asset!
