TheAssociate Patient Care Coordinator is responsible for thecompletion of set processes and protocols. Works cooperatively withall members of the care team to support the vision and mission ofthe organization, deliver excellent customer service and adhere toLean processes. Supports the teams in meeting financial, clinicaland service goals.

Schedule:

Monday through Friday between thehours of 8:00 am to 8:00 pm. (to work one night per week until 8:00pm) Two Saturday per month between the hours of 8:00 am to 4:00 pm.

This is a 37.5-hour work week to be determined by the supervisor.

PrimaryResponsibilities:
  • Obtain accurate and updated patientinformation, such as name, address, insurance information
  • Performinsurance verification on the date of service
  • Obtain patientsignatures for required documents
  • File and maintain medicalrecords
  • Confirms and schedule appointments
  • Follow the CashSecurity policy and procedures according to Optum Medicalguidelines
  • Perform referral documentation promptly
  • Performscertain follow-up services for patients in a prompt and courteousmanner, such as scheduling specialist appointments, schedulingfollow-up appointments
  • Adhere to the standards identified viaSparq regarding Optum Employee Policies
  • Work cohesively withfellow employees to achieve specific team goals
  • Comfortableworking in high pace environment
  • Assure the continuity of carethrough scheduling and tracking systems
  • Provide effectivecommunication to patient / family team members and other healthcare professionals as evidenced by documentation, case conferences,communication notes, and evaluations
  • Comply with administrativepolicies to ensure quality of care
  • Demonstrate precision andefficiency in scanning documents and monitoring the fax server,retrieving and / or scanning documents and assigning to theappropriate electronic chart
  • All employees are expected to keepabreast on current medical requirements relevant to their position,which includes maintaining patient confidentiality and abiding byall HIPAA and OSHA requirements

Required Qualifications:
  • High SchoolDiploma/GED (or higher)
  • 1 years of customer service or healthcarerelated experience
  • Intermediate level of computer proficiency(multi-tasking through multiple applications including MicrosoftOutlook, Excel, and Word)
Preferred Qualifications:
  • 1 years ofexperience working in medical front office/Urgent Care positionperforming duties such as scheduling appointments, checkingpatients in/out, insurance verification, collecting co-pays, andmaintaining medical records
  • Experience working with an electronichealth record (EPIC)
  • Knowledge of Medical terms
Soft Skills:
  • Ability to work independently and maintain good judgment andaccountability
  • Ability to multi-task and prioritize tasks to meetall deadlines
  • Ability to work well under pressure in a fast-pacedenvironment
  • Demonstrated ability to work well with health careproviders
  • Strong organizational and time management skills
