About New Horizon Counseling Center:

New Horizon Counseling Center (NHCC) is a leading non-profit behavioral health organization licensed by the New York State Office of Mental Health (OMH), and New York State Office for People with Developmental Disabilities (OPWDD) and New York State Office of Alcoholism and Substance Abuse Services (OASAS). New Horizon Counseling Center is seeking a full time, licensed social worker (LMSW, LCSW, LMHC) with excellent clinical, communication, documentation and organizational skills and experience providing clinical services to children, adolescents and adults for this unique opportunity as the Patient Referral Coordinator in its Central Intake Department. As the position of Patient Referral Coordinator does not require full-time responsibilities, the applicant will also maintain a small caseload of clients in an outpatient clinic. The Patient Referral Coordinator will serve as the initial contact with hospitals and other providers who wish to refer individuals for behavioral health and/or substance use services. The Patient Referral Coordinator will ensure that referrals are comprehensively and clinically screened and lead to efficacious and timely admissions. The Patient Referral Coordinator will work with the Intake team in assessing any referrals presenting as high risk, in crisis or requiring special needs. The responsibilities of this position will include the ability to:

• Function as initial contact for all referring agencies in the community.
• Assist clients with non-active Medicaid through initial entitlement process.
• Complete client detail intake (pre-screenings, Medicaid eligibility, Health Homes consents and intake assessment.
• Maintain good working relationships with all referring staff from hospitals and agencies throughout the served communities.
• Work closely with our Clinical Staff, Billing Department, Business Development, Care Coordination Teams and with the Medical Practice located in our building to share and refer clients.
• Set up Patient accounts and schedule clients through our Electronic Medical Record using the Accumed Program.
• Retrieve clinical documentation on all existing clients that are hospitalized and obtain discharge paperwork when clients are released.
• Maintain filing system and electronic documentation.
• Exhibit a friendly, enthusiastic and engaging demeanor with the ability to understand and explain all our programs in detail.
• Perform other duties as requested. As a licensed social worker with a small caseload, the applicant will be responsible for:
• Completing screening and intake assessments.
• Conducting individual, family and group psychotherapy (as needed) that is focused and goal oriented as clinically determined.
• Development of treatment plans.
• Crisis intervention.
• Formulating discharge plans.
• Participation in an integrated team approach to treatment and recovery.

Bilingual is a plus!
