PURPOSE OF POSITION

Reporting to the Director of Operations, the Assistant Director of Central Scheduling (Level III) provides oversight and supervision to clerical staff in customer service and scheduling for the Certified Home Health Agency (CHHA). As part of the leadership team, this position ensures that the team delivers quality customer service and timely care scheduling, guided by evidence-based practices. Additionally, the Assistant Director will train staff, stay updated on industry trends, and ensure compliance with relevant regulations and guidelines. Duties & Responsibilities 1. Directs, coordinates, and evaluates the delivery of Home Health customer service. 2. Liaises with schedulers, TCNs to coordinate patient referrals, patient care and patient assignments and assists in determining the patient’s eligibility for home care. 3. Monitors clinician staff assignments, caseloads, service utilization, and team productivity. 4. Ensures all case prioritization and scheduling is timely and accurate based on physician orders. 5. Participates in nursing and clerical orientation where indicated. 6. Evaluates and ensures competency requirements including annual evaluations are met for all direct reports. 7. Performs customer service and scheduling functions for the purpose of evaluating direct reports and may be asked to perform staff functions in times of increased demands or scheduling issues. 8. Identifies appropriate staffing patterns and needs of assigned staff based on productivity and team census. 9. Provides updates on census, staffing, team productivity and other reports as requested. 10. Responsible for performance evaluations and disciplinary action of all assigned staff. 11. Coordinates appropriate communication and documentation between patients, agency, medical professionals, and referral sources. 12. Performs case conferences and case load reviews for assigned staff as needed but minimally every two weeks. 13. Assists with assigned performance improvement data collection. 14. Ensures compliance with state and federal home health regulations as well as NYC H+H Home Care Policies and Procedures. 15. Participates in QI meetings/Quality Committee meetings and clinical record review when needed. 16. Participates in state or regulatory body surveys as needed. 17. Demonstrates sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations. 18. Participates in case conferences, utilization review and utilization management of admitted cases. 19. Conducts case load reviews and chart audits on a weekly to bi-weekly basis for all clinicians assigned to their team and develops interventions to correct identified deficiencies. 20. Ensure care delivery is rendered using the nursing process, and based on DOH/CMS and billing guidelines. 21. Monitors key metrics on the clinical and operational dashboard in EMR and performs data collection activities and related data analysis of the home care program. 22. Follows through on complaints and grievances as per agency policy and conducts chart reviews as needed to ensure issues are addressed timely and thoroughly. 23. Continuously analyzes work processes and makes suggestions for improvement. 24. Assigns staff to Interdisciplinary Team, review care delivery and promote interdisciplinary management of patients admitted to the home care department. 25. Works with Interdisciplinary Team and other Directors/Assistant Directors (Social Work and Rehabilitation) to develop appropriate interventions in order to achieve clinical and functional goals. 26. Assures the formulation, implementation and updating of an individualized care plan for each patient admitted to the

CHHA. 27.

Provides ongoing assessment of patients to ensure patient safety while on the home care program. 28. Reports all suspected compliance issues to Community Care Executive Leadership, Compliance Officer /Hotline. 29. Participates and provide training and continuing education as necessary. 30. Perform other duties as assigned. Minimum Qualifications Associate’s Degree in Business Administration, Public Health Administration or equivalent field and five (5) years of satisfactory experience, of which three (3) years shall have been in a supervisory or teaching capacity; or Bachelor’s Degree in Business Administration, Public Health Administration or equivalent field and three (3) years of satisfactory experience, of which one (1) years shall have been in a supervisory or teaching capacity.

KNOWLEDGEABLE IN

Knowledge of CHHA regulatory requirements. Strong communication skills (both written and verbal) Ability to thrive in a fast-paced, highly collaborative environment .
