Full-Time Direct Hire Job Responsibility :

• Supervise staff through chart reviews, case discussions, performance evaluations, and training
• Ensure timely and accurate documentation and compliance with Health Home guidelines
• Conduct client eligibility assessments and oversee care plan development
• Perform quality assurance and continuous quality improvement activities
• Submit internal/external reports and maintain accurate data in multiple systems

Requirements :

• Advanced degree in a related field with 1 year of supervisory experience and 3 years of relevant work, or a bachelor’s degree with 2 years of supervisory experience and 4 years of relevant work
• Experience with target populations (SMI, SUD, HIV/AIDS)
• Familiarity with NYS Health Home preferred
• UAS-NY certification required within one month of hire
• Required immunizations and tuberculosis testing
