Job Summary The Supervisor Case Management – SNH is responsiblefor the oversight of the case management staff’sauthorization/coordination/utilization and provision of memberservices. Duties include the supervision and mentoring of the casemanagement staff to ensure quality, efficient and comprehensivecare to our diverse member population. Provides direct oversight ofthe case manager’s daily operations of utilization review practice,processes and procedures ensuring accurate member care needs aremet and documented per the managed long term care program (MLTC)insurance regulations. Core Job Responsibilities Coordinates thedaily work within the Case Management function. Relays anddistributes work assignments and monitors work for quality andcompleteness. Ensures that the Case Management team workseffectively and efficiently with the interdisciplinary health careteam to meet patient, departmental and organizational goals.

Provides education and training to assist with the day to dayoperation of the Case Management department. Assists withidentifying over-utilization of services and reports trends to CaseManagement leadership. Ensures that the Case Management teamadheres to Conditions of Participation and all regulatoryrequirements that impact patient rights. Practices in accordancewith New York State Practice Act and applicable regulatorystandards. Engages in quality improvement initiatives includingcontinuing education, participation in system wide and unitspecific teams, identifying nursing practice areas for qualityimprovement and safe outcomes; maintains a cooperative relationshipamong the health care team by communicating information; respondingto requests; building rapport; participating in team problemsolving. Provides service excellence to patients and theirfamilies/visitors that meets or exceeds patient expectationsthrough offering empathetic and compassionate patient-centeredcare; professionally manage patients, families and care givers asneeded and concerns using positive service recovery methods;Responds to patient/family requests and makes active referrals tosubject matter expert/decision maker who can fulfill patientrequests; Provide culturally sensitive care. With leadership andHuman Resources guidance, provides input to or makesrecommendations on employee hires, performance reviews, transfers,promotions, salary changes, disciplinary actions, terminations andsimilar actions. Resolves grievances and other staff problems in afair, timely and consistent manner also in conjunction with HR.Ensures understanding and compliance with all MVHS policies andprocedures and regulations. Performs related duties as assigned.

Education/Experience Requirements

REQUIRED

Registered Nurselicensed in State of New York, Bachelor’s Degree in Nursing orhealth-related field Minimum of 1 year medical surgical experience.3 years Case Management experience. Ability to travel to work sitesand member homes within the community. Proven ability to lead,guide and motivate others. Proficient with MS Office and

EHR.PREFERRED

Prior supervisory experience Licensure/CertificationRequirements

REQUIRED

Licensed and currently registered topractice as a Registered Nurse in New York State.
