What You’ll Do:
Oversee Care Transitions :
Collaborate with hospital discharge planners, physicians, and interdisciplinary teams to develop personalized care transition plans for patients, ensuring a smooth and safe transition from hospital to home.
Conduct Home Visits :
Assess the health, living environment, and support systems of patients, making sure that the care provided is tailored to their individual needs and goals.
Direct Care :
Offer hands-on care, including medication management, wound care, and support with daily activities, while educating patients and families on chronic condition management.
Lead & Mentor :
Supervise and guide a dedicated team of nurses, personal care aides, and home health aides, ensuring that they deliver top-notch care in the home setting.
Quality & Compliance :
Ensure all care provided meets high standards, staying current with best practices and regulatory changes to continuously improve patient outcomes.
Collaborate & Educate :
Work closely with other healthcare professionals to create care plans and provide education that empowers patients and families to take control of their health.
What We’re Looking For:
Registered Nurse (RN) : You should have a degree from an NLN-accredited nursing school, with a preference for a BSN.
Experience :
At least 3 years of clinical nursing experience, with 1-2 years in home care or a similar healthcare setting. Previous experience with care transitions or case management is a plus.
Skills :
Strong clinical assessment, critical thinking, and communication abilities. You must be able to manage multiple patient cases effectively.
Licensing :
A current, active, and unrestricted RN license in New York State is required. Bilingual in Chinese is preferred.
