Job Description

The Continuum of Care Supervisor is responsible for supervising the Case Management & Social Work Services staff. One must possess the ability to assess and reasonably manage variables contributing to the reduction of length of stay (LOS) and cost per case through resource management. The Continuum of Care Coordinator is the key link between the HIM department, medical and nursing staff, compliance, clinical management, patient progression, gathering and reporting of utilization data, community outreach and as a resource in the prevention of readmissions. This person in this position will ensure clinical documentation adequately reflects resource management and severity of illness of the patient population(s). Extensive knowledge of all CMS regulations and conditions of participation are essential.

Key areas of responsibility:

+ Supervision of Case Management & Social Work Services Staff + Oversees and assists with staff scheduling and coordination + Oversees daily census and length of stay management; including progression of care + Provides discharge planning expertise + Coordinates Weekly Complex Case Rounds and follow-up + Assists with employee evaluations + Assists in coverage for weekends and holidays on monthly basis.

EDUCATION AND EXPERIENCE REQUIREMENTS

+ Active NYS RN Licensure and BSN Degree completed or in progress (completion within three (3) years). Masters Degree in a related field is preferred. + Candidates must have a minimum of 2-3 years leadership experience in Case Management, Utilization Management and/or Quality Assessment, or a minimum of 3 years leadership experience in a related field. + Eligible for or certified in Case Management is desirable.

PHYSICAL REQUIREMENTS

The position is located indoors under normal working conditions. Should be able to push/pull 25 lb., lift/move 15 lb. from floor to table, be able to perform moderately difficult manual manipulations such as using a key board, writing and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to sit at a computer terminal or work station for a prolonged period of time in addition to being able to squat, stand and walk for a reasonable length of time and distance. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language.

PRIMARY RESPONSIBILITIES OF THE POSITION

+ Ensure the administrative component for CMSW department. + Ensures compliance to budget throughout the year, including responding to internal and administrative changes. + Ensures that all human resources procedures are followed including hiring, discipline, evaluations and grievances. + Coordinates overall department operations. + Responsible for authorizing, reviewing and revising appropriate policy and procedures. + Communicates effectively with all levels of staff, between departments and entities acting as a liaison to facilitate info sharing. + Ensures productivity through appropriate use of coaching, mentoring and benchmarking data. + Completes and adheres to all hospital wide financial initiatives/procedures. + Monitor quality of all specialties within CMSW department. + Collaborate with staff, analyze departmental data, completes annually summary and develops future plans: + Identifies key aspects of services, and takes the necessary steps to continuously improve. + Reviews daily census for patients with and increased length of stay and oversees appropriate response + Responsible for coordination of processes including Utilization Management (UM), meetings, Committee reports, functions and minutes. + Responsible for ensuring best practices of Utilization Review, Case Management, & Social Work Services following established procedural guidelines + Works closely with medical staff leadership, clinical departmental chairs and committee chairs to ensure appropriate medical staff involvement in institutional resource management efforts. + Utilizes all hospital and physician data to review and analyze LOS, one day stays, observation patients, Inpatient MRIs, top DRGs, and transfers to other facilities and readmissions. + Responsible for accruing accurate collection of Avoidable Days, Targeted DRG goals and reporting the data to the UM Committee. + Collaborates with staff to discuss tools and techniques to reduce avoidable days and improve utilization. + Responsible to work and collaborate with the HIM department in reviewing charts for the assignment of the appropriate discharge disposition codes. The results of the chart audit will be reported to the UM Committees. + Ensures the growth / development CMSW staff/dept + Encourages and provides support to staff to be responsible for their overall professional development. + Keeps current by research, networking, joining associations and professional organizational groups, other departments, etc. + Works to support all hospital departments by providing technical assistance, direct support, information and responsiveness to issues. + Sets a collaborating environment by creating and maintaining a team approach. + Adheres to hospital policies and procedures related to: + Mandatory education + Annual health assessment + Licensure verification Ellis Medicine is committed to creating a diverse environment and is proud to be an equal opportunity employer. Salary Range:

$ 31.50-47.26 /hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
