The Patient Care Coordinator is a flexible team memberwho works to support nurse practitioners (NPs) and physicianassistants (PAs) as well as our patients. This role assists in carecoordination activities, administrative tasks and supports ourpatients and providers. This position is full time, Monday
- Friday. Employees are required to have flexibility to work any ofour 8-hour shift schedules during our normal business hours of8:00am
- 5:00pm.
It may be necessary, given the business need, towork occasional overtime. We offer 2 months of paid training. Thehours of the training will be based on schedule or will bediscussed on your first day of employment.
PrimaryResponsibilities:
- Support interdisciplinary care team with overall member coordination at the direction of RN or NP/PA
- Dataentry and member chart management for: New members, post hospitalvisits, quality, labs and medications
- Obtain medical recordsfollowing hospitalizations or physician appointments

RequiredQualifications:
- High school Diploma / GED OR equivalent workexperience
- Must be 18 years of age OR older
- 3 years of clinicalor medical record experience. Strong knowledge of medicalterminology and EHR platforms
- Access to reliable transportationthat will enable you to travel to patient locations
- Ability totravel up to 100% of the time
- Must reside in Berks or Lehigh, PACounty
- Ability to work full time, Monday
- Friday. Employees arerequired to have flexibility to work any of our 8-hour shiftschedules during our normal business hours of 8:00am

Itmay be necessary, given the business need, to work occasionalovertime.
Preferred Qualifications:
- Certified nursing assistantor medical assistant certifications The hourly range for this roleis $19.
86 to $38.85 per hour based on full-time employment. Pay isbased on several factors including but not limited to local labormarkets, education, work experience, certifications, etc.
