Position Summary The Director of Care Management helps develop and implement care management programs, manages protocols, policies, and departmental strategy in consultation with senior leadership. This role also involves leading and mentoring a care management team, collaborating with other healthcare professionals, and ensuring efficient coordination of care of discharging clients. Relationships and Contacts Within the organization: Initiates and maintains frequent and close working relationships with staff, clients, families, and peers throughout the organization. Outside the organization : Initiates and maintains strong professional relationships with clinical providers, referral partners and vendors, as needed. Essential Responsibilities Hires, trains, supervises and supports Care Managers at designated STR locations. Attends morning Network Flash calls daily, contributes relevant information with the cross functional team and keeps Care Managers updated on new/changing details. Directs aftercare planning processes and implementation for all clients admitted to STR Network, in collaboration with Clinical Leadership and Business Development. Attends site clinical team meetings, as available. Collaborates with Business Development to tour at external partner sites, educating clinical partners about STR’s Care Management program. Collaborates with Business Development and Executive Directors to support client, family, and clinical partner tours at STR Network locations. Ensures EMR is accurately and regularly updated with all relevant contact information for referring clinical partners. Conducts regular audits for each designated location to ensure accuracy of information and timely follow up actions completed; clinical partnership communication, family/client communication, discharge plans, and after care plans. Tracks and reports on incoming and outgoing professional referrals at each location. Maintains close professional contact with high need/high yielding accounts; EG EAPs, Insurance Carriers, Professionals, Hospitals, etc Conducts regular follow-up outreach with clients transferred to other facilities. Ensures care managers enroll discharged clients into alumni communication tools and groups, complete satisfaction surveys and follow up Google Reviews Ensures weekly alumni channel communication and platform are updates encouraging alumni connection and engagement. Additional Responsibilities Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends. Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations. Attends all supervisory trainings within assigned time frames as required by the facility. All Other duties assigned Requirements Education and Experience Must have a minimum of 2 years working directly as a Care Manager or Outreach Coordinator. One year of direct supervision and bachelor’s degree is preferred. Physical Requirements While performing the duties of this job, the employee will be required to communicate with peers/general public, clients and/or vendors. Job performance will require the ability to move throughout the building as well as sit or remain stationary for extended periods of time. While performing the duties of this job, the employee may be required to talk or hear, sit, stand, walk, reach, climb or balance, stoop, kneel, crouch or crawl, taste or smell. Care Planning and Intervention – demonstrates the ability to develop and implement evidence-informed care plans that promote clinical excellence throughout the client-centered continuum of care. Care plans outline necessary steps, identified collaboratively with the care team, to achieve objectives identified in the biopsychosocial assessment. Leverages evidence-informed, clinically excellent treatment interventions. Advocacy – Demonstrates the ability to appropriately advocate for the needs and interest of clients and families; promotes clinical excellence through quality improvement initiatives, improved outcomes, access to care, and service delivery. Interdisciplinary and Interorganizational Collaboration – demonstrate the ability to promote cross functional collaboration among team members and referral partners to support and enhance clinical service delivery and support.
