ABOUT THE JOB
We are seeking a motivated Director of Case Management of for a 13-week assignment at an Acute Care Hospital in Johnstown, PA.
QUALIFICATIONS MUST
have director experience at hospital >150 bed, managing large teams (approx 20) Acute, LTAC, SNFs, and TCU Experience (CM Director will need to keep Transitional Care Unit full) MUST have EPIC experience, InterQual, discharge planning, Conditions of Participation, CMS guidelines, required. Will need to provide education and guidance with providers- this is a younger group who needs guidance to grow their confidence w/ their discharges, getting discharge done in the morning, etc.
