Position Summary:

The Multi-Facility Admissions Coordinator is responsible for managing the referral and admissions process across multiple skilled nursing or senior care facilities. This position plays a critical role in census development by coordinating patient transitions from hospitals or the community into the appropriate facility within the network. The coordinator ensures that all admissions are handled promptly, efficiently, and in compliance with regulations and internal policies.

Key Responsibilities:

Referral Management Receive and log referrals from hospitals, case managers, and other referral sources. Coordinate with the Director of Admissions and central admissions nursing team, who determine clinical appropriateness and acceptance. Once approved, process admission paperwork and collect necessary documentation from referral sources and families. Accurately enter patient demographic and insurance information into the EMR system (e.g., PointClickCare). Facility Coordination & Room Readiness Communicate bed availability across all assigned buildings. Coordinate with onsite admissions, nursing, and administrative staff. Ensure rooms are clean, set up, and fully prepared for incoming admissions, including supplies, linens, and personal touches as needed. Electronic Documentation & Data Entry Complete all electronic admission paperwork with accuracy and efficiency. Upload clinical documentation and referral packets into the EMR system. Maintain organized digital records in compliance with HIPAA and company policy. Customer Service & Community Engagement Provide exceptional customer service to residents and families during the admissions process. Conduct facility tours for prospective residents and families. Assist Director of Admissions to Represent the facility at community events and health fairs to promote services and build referral relationships. Onboarding Support Assist the Director of Admissions with all aspects of onboarding new admissions and readmissions. Coordinate closely with the clinical, housekeeping, dietary, and therapy teams to support a smooth transition for incoming residents. Communication & Reporting Maintain accurate records and reports in the admissions database. Serve as a liaison between referral sources, facility teams, and families. Provide regular updates to leadership on census, referrals, and outreach activities. Additional Duties Assist with initiating or following up on insurance authorizations and prior approvals, especially for managed care. Communicate as needed with payers to support central admissions or billing teams. Follow up as needed with hospital case managers or discharge planners to confirm next steps or status updates. Help maintain an accurate bed board or daily census report across facilities. Confirm family preferences, dietary needs, or personal requests prior to admission. Coordinate transportation for incoming residents, including with families or external vendors. Prepare and deliver welcome packets to new residents and support their initial orientation. Help maintain marketing materials and admissions packets; assist with outreach logistics when needed.

Qualifications:

High school diploma or equivalent required Minimum of 1 years experience in admissions, customer service or patient representative Some Knowledge of Medicare, Medicaid, Managed Care, and insurance verification processes. Strong communication, time management, and multi-tasking abilities. Proficient with computers EMR systems such as PointClickCare; Ability to work independently and prioritize across multiple facilities.

Schedule & Travel:

Full-time, Monday through Friday. Some weekends may be required. Travel between The Gardens at Orangeville and The Gardens at Millville, some local travel to customers may be required. Mileage reimbursement for travel between the 2 buildings or any local travel.
