Behavioral Health Patient Care Coordinator (Masters Level) 2.8 2.8 out of 5 stars 740 East State Street, Sharon, PA 16146 We are seeking a compassionate and dedicated Patient Care Coordinator to be a part of the multidisciplinary psychiatric treatment team. The PCC is directly involved with psychiatric inpatients, completing psychosocial assessments, treatment plan development, contact with family and collateral supports and arranging all aspects of the aftercare disposition. The PCC additionally provides therapeutic support to patients in both individual and group therapy formats. Qualifications Master’s Degree in Psych, Counseling, Social Work on related field.

Minimum of 3 years experience in Behavioral Health Strong understanding of adult, child and adolescent DSM V treatment. Excellent organizational and communication skills Familiarity with healthcare regulations and patient confidentiality standards
