This includes supporting residents with a range of services such as: Direct, supportive, and personal care Assistance with activities of daily living (ADLs) Medication distribution Documentation and reporting Encouraging resident independence and honoring personal choice All responsibilities will be carried out in a manner aligned with our Person-Centered Care philosophy.
Minimum Job Qualifications and Requirements:
Must have a valid South Carolina
LPN / LVN
License Ability to act independently in both routine and emergency situations Courteous and diplomatic communication with residents and staff Flexibility to work varied hours, including holidays Strong team collaboration and reliability Professional, respectful, and positive demeanor Ability to maintain confidentiality Strong organizational and customer service skills Ability to work independently with minimal supervision Time management skills to meet deadlines Compliance with all company policies and procedures Scheduling experience a must ADP & Point Click Care a plus
Essential Job Duties and Responsibilities:
Assist with daily operations of care staff, including scheduling and interviews Supervise community Med-Techs Monitor resident healthcare routines Complete and submit incident reports according to state guidelines Assist with ADLs such as dressing, bathing, hygiene, and grooming Care for ADL devices such as eyeglasses, hearing aids, and contact lenses Maintain knowledge of state regulations related to resident care Observe and report changes in resident status to the Health Services Director Conduct room checks and resident rounds Monitor and report environmental safety hazards Respond promptly to resident calls, emergency pull cords, and requests Use tactful communication in sensitive or emotional situations Maintain thorough daily documentation of resident activity and condition Report emergencies and changes to the Health Services Director and Executive Director Follow up on complaints or concerns with appropriate parties Represent the community in a professional and friendly manner Maintain all necessary certifications Monitor and manage use of care and office supplies Maintain accurate resident census records Encourage resident involvement in activity programs
Other Job Duties and Responsibilities:
Maintain communication with residents’ physicians Document and manage meaningful resident interactions and transitions Ensure all documentation complies with state and facility requirements Support the Wellness Department in ensuring resident safety and needs Help new residents acclimate and coordinate with other departments Assist with family tours and explain the case manager’s role Evaluate residents and update care plans as needed Complete long-term care insurance documentation Help residents apply for Veterans Affairs Aide and Attendance Review shift notes, incident reports, discharge papers, and medical orders Provide written or verbal updates on resident status Share on-call responsibilities with the Health Services Director Use personal vehicle for off-site assessments and screenings Perform duties for agreed-upon salary without accepting gifts or extra payment Serve as a professional role model with a cooperative and positive attitude Apply today and help us create a community where compassion and connection come to life!
