Adoration Home Health and Hospice Overview:
The Clinical Manager is responsible for the quality of careprovided for patients in their area of responsibility. Collaborateswith other disciplines, departments, and support services to assureand improve patient outcomes. Demonstrates professional managementand leadership in clinical practice. Works collaboratively with andtakes direction from clinical support team. Works with a highdegree of independence to ensure optimal clinical, satisfactionoutcomes are achieved in a timely and efficient manner. Ensurespractice standards, company policy and regulatory requirements aremet and drives efficiency in care delivery processes. The ClinicalManager is accountable for overseeing the day-to-day clinicaloperations to ensure quality of care & services, regulatorycompliance, support of business development & growth, achievementof key performance indicators, and quality outcomes. Responsiblefor the oversight, direct supervision and development of cliniciansand direct-care staff.
Responsibilities:
Conducts/participates in quarterly quality improvement meetings,reviews pertinent available data, identifies trends and overseesthe implementation of improvement plans Collaborates with Branch Director in the implementation of qualityimprovement activities and corrective action plans Provides feedback to Operations management regarding necessarychanges and revisions relating to care provision Accountable to ensure all clinical staff appropriately andsuccessfully complete orientation and all required in-services Responsible for the oversight of and response to external andinternal surveys and audits within assigned area relating to thequality of care and clinically related KPIs in collaboration withoperations leadership Acts as a resource and has a working knowledge of payerrequirements relating to the provision of care and documentationguidelines Completes documentation and chart reviews to identify variances instandardized care and care processes and to identify areas forimproved performance related to patient care and theorganization Holds case conferences to ensure oversight of care, coordination ofservices and that standards are met Ensures documentation standards are met per company standards andto support quality standards, coverage, and reimbursement Ensures that care is coordinated with all involved in patientcare Identifies variances in standardized care and care processes toidentify areas for improved performance related to patient care Ensures compliance with regulatory, payer and companyrequirements Facilitates the development of performance improvement actionplans, based on available data (e.g., survey results, QAPI andclinical record reviews) Accomplishes staff results bycommunicating job expectations; planning, monitoring, andappraising job results; coaching, counseling, and discipliningemployees; developing, coordinating, and enforcing systems,policies, procedures, and productivity standards Oversees and directs the selection and hiring of clinical staff Conducts interviews and makes hiring decisions Ensures staff competency and performance Ensures assigned staff are evaluated fairly and timely inaccordance with company performance evaluation procedures Ensures direct, on-site supervision for all staff per companyrequirements Responsible for achieving organizational performance and retentiongoals among clinical staff Serves as a resource in assisting the field staff with problemsolving to provide the most appropriate care for the patient Serves as a coach, mentor, and role model to all professional anddirect-care staff Ensures adequate numbers of preceptor staff to provide developmentand mentoring of staff clinicians and direct-care staff At least one preceptor clinician is trained to provide back up forthe clinical manager position Other duties as assigned – may be asked to perform patient visitsand perform on-call/after-hours duties in the event of staffingissues
Qualifications:
Bachelor’s Degree preferred in Nursing or Physical Therapy from anaccredited school of nursing or therapy Licensed Registered Nurse (RN) or Physical Therapist (PT) in goodstanding and currently licensed by the State Less than two years clinical experience, and at least one year inrelated management/supervisory role in home health care Current CPR certification Must meet all agency requirements for pre- employment as requiredby the company and/or State regulations Ability to use company documentation systems Ability to communicate (verbally and written) with all levels ofpersonnel, internal and external to the company Ability to work independently as well as part of a team Capable of working responsibly with highly confidential About our
