Job Description
Director of Community Health and Impact2, Prevention and Wellness, Full Time, First Shift 3.7 3.7 out of 5 stars Greenwood, SC 29646 This includes in detail but is not limited to: AccessHealth Lakelands, SRH/ECH Healthy Outcomes Plan, SBIRT programing, Prevention and Wellness services, Community Health Services Mobile Clinic, Community Health Needs Assessment (CHNA), Community Benefit Reporting (CBR), CMS Health Equity Framework and Measures, and all community outreach services for education and coordination, etc. This includes identifying opportunities and needs as well as guiding teams in strategic planning, project management, and financial management of implementation and operation of those services. Strives to uphold the core values of the organization leading by example daily. Promotes the corporate culture externally in various community and civic functions. Places an emphasis on the leadership development of their area of responsibility. Consistently coaches and develops team members to foster a spirit of collaboration, diversity of thought, and unity to serve our patients, team members and the communities we serve. Basic (Required)
Qualifications:
These specifications are general guidelines based on the minimum experience normally considered essential to the satisfactory performance of this position. The requirements listed below are representative of the knowledge, skill Director of Community Health and Health Equity and/or ability required to perform the position in a satisfactory manner. Individual abilities may result in some deviation from these guidelines. Bachelor’s degree in Business Administration, Nursing, Public Health Social Work or Health Care Administration Must be legally authorized to work in the United States without company sponsorship A minimum of Five years direct leadership in the Healthcare or related arena required. Prior experience with facilitating community healthcare access strongly preferred. Prior experience with grant writing required (in the Healthcare arena preferred). Prior experience with local community interactions and familiarity with the regional and state healthcare environment, physician network, human service organizations and other related entities preferred.
Preferred Qualifications:
Master’s Degree Education in public health/healthcare preferred Not-for- Profit program/organization leadership experience strongly preferred.
