Perinatal Nurse Coordinator, Access Health Lakelands 3.7 3.7 out of 5 stars Greenwood, SC 29646 Responsibilities include identifying and managing high-risk patients, conducting clinical chart audits, supporting remote patient monitoring, and providing direct patient education and coordination. The nurse will work collaboratively within a multidisciplinary team to address both medical and social needs, ensuring comprehensive care for expectant mothers across Self Regional Healthcare’s service area. This role may also involve conducting home visits to assess patient needs and provide clinical support.
Basic Qualifications Licensure:
Current LPN or RN license in South Carolina.
Experience:
Minimum of two years of experience in maternal health, OB/GYN, community health, or chronic disease management. Familiarity with managing patients with hypertension, diabetes, and high-risk pregnancy conditions.
Technical Skills:
Experience with electronic health records (EHR), preferably EPIC. Ability to collect, analyze, and report patient data for program evaluation.
Interpersonal Skills:
Strong communication and patient education skills. Ability to work collaboratively with interdisciplinary teams. Cultural competency and experience working with diverse populations. Preferred Qualifications Certification in Care Management (CCM) or Chronic Disease Management. Experience with remote patient monitoring (RPM) technologies.
