Position Summary:
Cooperative Health is seeking an innovative, enthusiastic individual with strong leadership, customer service skills, case management, communication, and Microsoft application skills (e.g. Excel spreadsheet). The position will focus and conduct activities including: rapid HIV/HCV(hepatitis C) test counseling, support, and education across Cooperative Health satellite sites, community outreach, educational presentations, outreach to reach clients in danger of falling out of care or lost to follow up, provide. Build department outreach awareness, thereby improving clients’ / staff capacity to use the health care system and improve health outcomes
Principles Responsibilities:
Conduct yourself in accordance with Cooperative Health community prevention and outreach protocols Document each outreach contact during or after each outreach excursion Responsible for tallying monthly outreach contacts and completing Cooperative Health paperwork Conduct educational presentations health related topics at schools and community locations including in-services to partner agencies Conduct rapid HIV/HCV testing in the community to include 25 patients a week in clinical-Cooperative Health sites and especially in non-clinical settings Conduct a minimum of twenty (20) community events to raise awareness of HIV, PrEP, STI’s, Hepatitis C and other related diseases every 6 months. Develop community partnerships with agencies. Investigate and locate patients who are in danger of falling out of care, lost to care and determine if and where they are engaged in care. Demonstrate understanding of Cooperative Health policies and procedures. Observes all policies and procedures for use of time-keeping system, including attendance, tardiness, proper clocking procedure Consistently maintains appearance and dress standards that are appropriate, safe, and in accordance with Cooperative Health standards and policy Attends Cooperative Health meetings as required and data entry as required Work on special projects, as assigned Employee must be able to work independently and collaboratively with a team approach Interacts with patients, physicians, staff, vendors, and visitors in ways that demonstrate mission and vision of Cooperative Health Must be able to obtain and organized inventory including rapid HIV/HCV testing supplies, promotional items and educational material. Must be able to lift 15 pounds Must be able to maintain and document encounters from rapid HIV/HCV testing, document in evaluation web, and maintain an active and upto date spreadsheet. Routinely participate in local, statewide and national virtual meetings, when needed Performs outreach activities to recruit people with HIV in targeted areas Targeted Rapid HIV/HCV testing will be conducted in service areas where new cases of high risk behaviors are indicated or “hot spots” Identified HIV positive individuals are linked and retained in care Documentation must be completed in the electronic medical record and /or Evaluation Web within 72 hours Ensure people who are positive are retained in care Maintain an active spreadsheet in real-time and case monitor/ case manage clients on the list. Ability to work flexible hours, including evenings and weekends Conduct street outreach a minimum of twice a week and distribute outreach materials Other duties as assigned
Education & Experience:
High school diploma or equivalent and four years of work experience in a medical care setting is acceptable. Supervisor Experience preferred Master’s degree preferred from an accredited college or university in Social Work or Counseling or Public Health or Health & Human Services Field. Bachelor’s degree accepted At least two years working in a clinical/community setting and/or successful community outreach/recruitment and volunteer coordination, or any equivalent combination of training and experience. Spanish speaking is a plus. Must have the ability to understand and respond effectively and with sensitivity. Experience working with diverse communities to address healthcare needs. Federally Qualified Health Center (FQHC) experience preferred demonstrated proficiency and comfort with technology; able to use MS Office (e.g., Word, Excel, Outlook, and PowerPoint), database management, webinar systems, and HTML email systems effectively and efficiently.
