What You’ll Do:
As a Patient Care Coordinator, you’ll serve as a key liaison between clients, families, and our care team—ensuring seniors receive personalized, high-quality care in a safe and supportive environment. Conduct initial in-home care consultations, assessing medical, personal, and social needs. Build and maintain trusted client relationships, proactively addressing concerns and anticipating changes in care needs. Perform regular client assessments, identifying safety risks and making professional recommendations. Provide nursing oversight for caregivers, including hands-on skills training and competency validation. Supervise caregivers and monitor ongoing care through scheduled home visits. Ensure compliance with all state regulatory requirements and documentation standards. Manage medication oversight and perform necessary nursing interventions. Educate and support families, helping them understand care options and resources. Collaborate closely with our care coordination and sales teams to ensure continuity of care and client satisfaction.
What We’re Looking For:
