JOB SUMMARY

Responsible for the day-to-day clinical operations, overall coordination, and administration of the hospice program and responsible for the compliance of clinical services with all state and federal hospice regulations. Exercise general oversight of the delivery of hospice care. Promotes cost-effective and clinically excellent hospice care. Manages professional growth and job satisfaction for the hospice interdisciplinary group (IDG) including the management functions of planning, organizing, directing, and controlling. Manages referrals and admissions to ensure prompt service. Maintains communication between hospice, hospital administration, and other hospital departments and care partners including nursing homes, assisted living facilities, group homes and physician practices.

GENERAL REQUIREMENTS

Adheres to the hospital and departmental attendance and punctuality guidelines Performs all job responsibilities in alignment with the core values, mission and vision of the organization Performs other duties as required and completes all job functions as per departmental policies and procedures Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs) Wears protective clothing and equipment as appropriate. Ensures clinical and organizational compliance with all state and federal hospice regulations. Actively participates in the hospice’s Quality Assurance and Performance Improvement (QAPI) program. Facilitates staff and Interdisciplinary Group (IDG) meetings and ensures the IDT members complete mandatory in-services and requirements and competency evaluations on time.

WORKING CONDITIONS

General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels. May be exposed to limited hazardous substances or body fluids, or infectious organisms. May be required to change from one task to another of different nature without loss of efficiency or composure. Periods of high stress and fluctuating workloads may occur. Long-distance or air travel as needed

EDUCATION REQUIREMENTS 4

year / Bachelor’s Degree in Nursing (Required) Master’s Degree in Nursing or Health Administration (Preferred)

CERTIFICATIONS AND LICENSURES
Required Certifications/Licensures:

Registered Nurse (RN) with current Georgia license, Hospice and Palliative Registered Nurses (RN) obtained within 3 years of hire
