Essential Job Responsibilities1. Job responsibilities include those listed in competency document.2. Translates the McLeod Health vision and goals into practical strategies. 5. Responsible for operational management of the Case Management Department. 6. Responsible for implementing processes that support and promote efficient patient throughput across the healthcare continuum resulting in optimal clinical outcomes, appropriate utilization management processes, total cost of care initiatives and care coordination activities. 7. Provides clinical management and leadership to staff in nursing roles with a goal of advancing the practice of Hospital Case Management.8. Utilizes and interprets data to identify factors allowing for rapid change/course adjustment to stay on target and achieve operational and program goals.9. Develops and implements standard work including regulatory requirements and operational policies and procedures for Clinical Case Management services and assists corporate educator with cascading educational materials.10. Understands and can implement rapid cycles of change which results in successful short‐term and large‐scale organizational change.11. Responsible for assessing clinical competency of staff and reinforcing skills supporting efficient patient throughput along the continuum of care.12. Complies with all applicable government regulations and contractual obligations relating to provision of Clinical Case Management services.13. Assists in the planning and development of an annual budget; analyzing variances; initiating corrective actions.14. Prioritizes work based on the needs of the organization and in accordance with the McLeod Health strategic plan.15. Directly responsible for leading and development of a high-performance team by participating in recruiting, selecting, orienting, and training of Case Management staff. 16. Motivates team members to success through a collaborative leadership style.17. Completes annual performance evaluations according to policy.18. Coordinates and oversees multi-disciplinary team care meetings to ensure appropriate throughput.19. Responsible for assessing, reporting, and communicating program status on a periodic basis to all program stakeholders.20. Develops and uses the LEAN system to continuously evaluate and address internal and external opportunities for improvement.21. Identifies high priority interventions to improve performance and actively drive measurable outcomes related to Clinical Case Management. 22. Works with Physician Advisor/Utilization Management program and Case Management leadership on assigned campuses for focused and prioritized improvement and compliance. 23. Works with Home Care Services leadership to assure smooth transitions of care and expedite discharge to appropriate cost effective services.
