Our patient coordinator team sits at the heart of our operations, connecting physicians, pharmacies, health plans, and patients with the industry-leading service we’re known for. We’re seeking a patient coordinator to join our Nashville team to help build and coordinate our community, onboard interested patients, and educate healthcare professionals, so we can continue in our vision to transform the care experience for people at home.? You will be responsible for contacting prospective patients and coordinating the onboarding process with our various pharmacy partners, as well as onboarding patients introduced to us through our strategic partnerships. Communication, sales, and organizational skills are key to thriving in this role. •While this is a remote position, we require applicants to reside in the Nashville, Tennessee area.
Key Responsibilities:
Calling consumers who have requested more info, or been referred, explain the service, verify coverage, and assist with onboarding. Liaison between enrolled patients and assigned pharmacy partners. Organize and track patient progress through the onboarding process. Seek to develop and maintain strong customer relationships while identifying new business. Help refine and design processes that allow for frictionless onboarding. Manage CRM pipeline and schedule to maximize opportunities. Ad hoc duties.

? Strong customer relationship skills. Highly organized and detail oriented. Ability to work in a fast-paced growing environment. Pharmacy, social work, or healthcare experience is a plus. Experience using a CRM is a plus. Excellent verbal and written communication skills with professional telephone presence. Superior organization and time management skills with the ability to multitask. Proficient in computer use and software programs.
Experience:
Health care, social work, customer care/service: 1 years (Required) Call center experience preferred.
