Line of Business Abode Care Partners Position Type Full-Time Our Company Abode Care Partners Overview The Regional Clinical Director of Abode Care Partners Primary Careleads the clinical operations, programs and activities for theassigned region and ensures the delivery of high-quality patientcare. This includes services spanning Skilled Nursing Facilities,Assisted Living, Group Homes, Independent Living and privateresidences. This position is a key member of the ClinicalOperations leadership team and will advise on and execute strategyfor the company in partnership with all partner team resourcesincluding professional development, human resources, quality andcoding, and value-based operations.

Responsibilities For assigned territory:

Lead and manage up to 30 AdvancedPractice Providers as direct reports.

Responsible for on-boardingand care model training of new Advanced Practice ProvidersMayprovide clinical coverage for unexpected call-offs, vacations, orspecial needs as indicated and requiredConduct professionaldevelopment and the annual performance, competency, and compliancereview processDrive performance and support retention efforts toinclude regular coaching and review of: Provider incentive structureProviderscorecard (coding, quality, patient experience, and productivityreporting) reviewsVisit distribution and workflowimprovementPerformance Improvement Plan creation and managementExecute on regional business, growth, and value-based strategiesfor respective operationsProvide account and relationshipmanagement in conjunction with the regional administrative partner:increase building penetration, serve as the escalation for theleaders for all facilities in your region, and engage with patientsand/or families to facilitate issue resolution. Provide outstandingcustomer service to all clients, patients, and families.

APRN, FNP, AGNP or PA At least three years of experience practicing in AssistedLiving, In-Home, Group Homes, Independent Living or Skilled NursingFacilities At least two years of nursing management experience in a healthcare industry Strong interpersonal and communication skills to effectivelycommunicate with Practitioners, office personnel, andpatients. Demonstrated ability to develop and implement clinicalinitiatives. Proven track record of improving patient outcomes. Computer literacy and knowledge of relevant healthcare andadministrative software Excellent analytical and problem-solving skills Organizational and time management skills Strong leadership, motivation, training, and goal-settingskills Strong financial acumen and budget management skills Ability to travel up to 30% in assigned region.
