Brief Job Summary :
The Mental Health Coordinator is responsible for the implementation of the Plan for individuals assigned. The Mental Health Coordinator is directly involved with supporting individuals in developing emotional, social, intellectual, and independent living skills necessary to live learn and work as active, healthy and productive members in their communities through interventions developed by behavioral health professionals.
ESSENTIAL JOB FUNCTIONS
1. Assess the individual, develop a treatment plan, and implement treatment goals and interventions. 2. Perform orally and written communication effectively across multiple platforms. 3. Complete a Daily Note prior to the end of the day that includes an overview of the individual’s day, their response to the group topic or activities that occurred. 4. Maintain a safe environment by keeping surroundings organized and free from clutter. 5. Create a positive environment by implementing meaningful activities that can improve or foster a positive social experience for the individuals. 6. Develop and implement a group curriculum for psychosocial rehabilitation program. 7. Must have the ability to facilitate 3 hour group session with individuals. 8. Participate in weekly team meetings to present new ideas or suggestions. 9. Meet monthly with psychiatrist to review the progress of individuals in psychosocial rehabilitation program. 10. Demonstrate the ability through verbal or written communication with multidisciplinary to integrate knowledge and methods from different disciplines to obtain knowledge to better serve the individuals.
