Job Description:
Are you passionate about making a positive impact in the lives of individuals facing challenges and in need of support? We are looking for a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing guidance, coordination, and advocacy to help our clients achieve their goals and improve their overall well-being.
Responsibilities:
Conduct comprehensive assessments to understand clients’ needs, strengths, and challenges. Develop individualized case plans in collaboration with clients, setting achievable goals and action steps. Provide support, resources, and referrals to assist clients in accessing essential services and programs. Monitor and evaluate clients’ progress and adjust case plans as necessary. Advocate for clients’ rights and needs, ensuring they receive the best possible care and services. Maintain accurate and up-to-date case records and documentation in compliance with agency standards. Collaborate with a multidisciplinary team to coordinate client care and support.
Qualifications:
Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related field. Relevant licensure or certification is a plus but not required. Strong communication and interpersonal skills. Empathy, patience, and a client-centered approach to service. Organizational and time-management skills. Ability to work effectively in a team-oriented environment. Knowledge of community resources and social services is advantageous.
