This person shall oversee and be responsible for all contracted care coordination (CC) activities for the CHOICES program as defined in this Contract, or in TennCare policy or protocol. This is a field-based position requiring the CC to travel, assess, create plans of care, and authorize LTSS services within prescribed timeframes. This team is responsible for the direct and day to day management of our LTSS members in Davidson and Williamson County. If you reside in Davidson or Williamson County, TN, you will have the flexibility to work remotely• as you take on some tough challenges.
Primary Responsibilities:
Travel, assess, create plans of care
Authorize LTSS services within prescribed timeframes

Required Qualifications:
Current, unrestricted LBSW, LMSW or LCSW licensure or current, unrestricted licensure as LPN 1+ years of clinical or case management experience
1+ years of experience in long-term care, home health, hospice, public health or assisted living
Driver’s License and access to reliable transportation to be able to conduct field visits
Preferred Qualifications:
Medicare/Medicaid experience
Experience with electronic charting
Background working in geriatric special needs
