Job Description

Overview Join our team as a Care Coordinator where you’ll play a vital role in ensuring patients receive seamless healthcare services from initial evaluation through treatment completion. This position requires exceptional organizational skills, healthcare knowledge, and a passion for patient advocacy. Key Responsibilities Serve as the primary point of contact for medical offices, insurance adjusters, nurse case managers, and patients Coordinate and manage comprehensive treatment plans, ensuring accurate documentation and scheduling Process treatment authorizations and promptly schedule approved appointments Review and respond to inquiries from stakeholders in a timely, professional manner Educate provider offices on required documentation protocols and submission guidelines Obtain and verify all necessary appointment documentation from healthcare providers Escalate complex issues to appropriate leadership channels when necessary Contribute to process improvement initiatives and system enhancement recommendations Participate in testing and implementation of system/process improvements Qualifications Prior experience as a Care Coordinator or Medical Assistant required (2+ years preferred) Strong understanding of healthcare coordination processes and terminology Exceptional communication skills, both written and verbal Advanced problem-solving abilities and analytical thinking Excellent multitasking capabilities while maintaining attention to detail Intermediate proficiency with Microsoft Office suite, particularly Outlook Experience with medical software platforms highly valued Contact center or customer service background a plus Ability to maintain strict confidentiality and adhere to security protocols Bilingual skills considered an asset Education High School Diploma or GED required Healthcare-related certification or coursework preferred
