Position Purpose:
- As a Personal Care Assistant at TrinityHealth PACE (TH PACE), you will play a crucial role in enhancingthe lives of our participants by providing compassionate care andsupport. Your responsibilities will include assisting with dailyliving activities, ensuring participant safety, and promoting anurturing environment that upholds our mission and values.

What You Will Do:
- + Provide personal care assistance to participants, includingtoileting, bathing, grooming, and meal assistance.
+ Monitor participant health and report changes to theinterdisciplinary team. + Support participant independence in activities of daily livingand assist with physical activities and exercises. + Maintain accurate documentation of services provided andparticipant progress. + Collaborate effectively with team members to ensure holisticparticipant care. + Escort participants to medical appointments and engage inrecreational activities.

Minimal Qualifications:
- + High school diploma or equivalent required. + CNA certificate required. + Own Vehicle and Insurance + Minimum one year of experience working with elderly or frailpopulations. + Understanding of state regulations and health standards. + Excellent communication skills and proficiency in MicrosoftOffice Suite. + Ability to handle multiple tasks with flexibility andadaptability.
