Full Job Description Overview:
DSPs play an important part in helping the people we serve live their best lives with meaning and dignity. DSPs are representatives of Amivie and are expected to create and maintain professional, positive, and nurturing relationships with person served and established support systems. It’s important that DSPs must be comfortable and willing to be a vital part of providing individualized care to those we serve. Shifts that need to be covered: Jacksonville 1. (Professional Crisis Management) (Restraint ‘ s & Take Down ‘ s) for therapeutic restraint holds and are able to go down to your knees with them if needed. DSPs provide assistance with Activities of Daily Living (ADLs), accompany our individuals to medical/dental appointments, community integration, drop off/pick up for home visits, execution of planned active treatment goals, electronic documentation, medication management, meal preparation, and interaction with residents. Responsibilities:
Provide assistance with Activities of Daily Living (ADLs); (i.e. bathing, grooming, brushing teeth, toileting, feeding, etc.) based on individual needs/person centered plan. Administer prescribed medication(s) to identified resident(s) upon completion of training. Safely transport resident(s) in company approved vehicles. (Vehicles can be a 6-15 passenger van depending on the program) Accompany the resident(s) as needed to medical/dental appointments, community integration, drop off/pick up for home visits (aka Therapeutic Leave), etc. Complete all required documentation in the electronic health record (EHR) timely. Prepare meals and snacks according to dietary needs and meal plans. Actively engage with resident(s) in their home and community Uphold, respect, and advocate for resident rights at all times. Ensure compliance with HIPAA regulations and maintain the confidentiality and privacy of resident information at all times. Provides staff coverage in other locations as needed. Adhere to company policies and core values to foster a professional and supportive environment. Establish and uphold appropriate professional boundaries in all interactions, ensuring that relationships remain ethical, objective, and focused on individual and organization goals. DSP’s are mandated reporters and legally required to immediately report any suspicion of abuse, neglect or exploitation. Ensure cleanliness of home and report any maintenance concerns immediately Must be proficient with technology. Must remain awake and alert for the duration of all shifts. Must be able to perform physical demands of the position (i.e. lifting, transfers). Other duties as assigned.
Training Requirements :
Ensure all certifications and training requirements and renewals are met on time to maintain compliance.

Minimum of High School Diploma or GED (General Equivalency Diploma) Must be at least 18 years old Valid driver’s license in good standing Must have a reliable transportation Must successfully complete all background checks Must successfully complete health screening (TB screening)
