Full Job Description Overview:

We are seeking a dedicated and compassionate Direct Support Professional to join our home care team. As a Support Professional, you will play a vital role in providing direct patient care and support to our consumers. Schedule:

Saturday & Sunday

• Flexible start times Provide companionship through conversation, recreational activities, and emotional support.

Support the consumer in safely participating in hobbies such as arts and crafts, light exercises, or community outings. Assist with mobility and transfers, including wheelchair support, under appropriate supervision. Maintain a safe and positive environment that respects the dignity and preferences of the consumer. Communicate effectively with the consumer using appropriate methods, including assistive technology if needed. Observe and report any changes in the consumer’s condition or behavior to supervisory staff. Document daily interactions, support provided, and any notable activities or incidents. Encourage participation in community inclusion and life enrichment activities. Comply with all organizational policies, confidentiality guidelines, and safety procedures.

Requirements:

Must be 18 years or older with a high school diploma or equivalent Must pass a Level 2 Background Screening & Local Law 2 years of employment reference and 3 personal references 1st

AIDE/CPR

Medication Administration Experience working with special needs

Required Training:

Zero Tolerance, HIPPA, HIV/AIDS, Direct Care Core Competencies, Electronic EMR Wihin 30 days of employment

Salary:

$17.00

• Proven experience as a Direct Support Professional or similar role
• Proven Experience working with people with an intellectual disability
• Knowledge of anatomy, medical terminology, and basic healthcare procedures
• Familiarity with home care, assisted living, or other healthcare settings is a plus
• Excellent communication and interpersonal skills
• Ability to multitask and prioritize tasks effectively
• Strong attention to detail and accuracy in documentation
• Compassionate and empathetic nature towards patients
