Full Job Description:

Complete Care Home Health Agency is currently seeking a dedicated and compassionate Medical Social Worker (MSW) to provide home-based social services to our patients. The MSW will work closely with patients, families, and the interdisciplinary team to support psychosocial needs, assist with community resources, and promote patient independence and quality of life. This role offers the flexibility of per diem visits, competitive per-visit compensation, and the ability to make a meaningful impact in the lives of patients and caregivers.

Responsibilities:

Conduct initial psychosocial assessments in the patient’s home Assist patients and families in understanding and coping with diagnoses, treatment plans, and lifestyle changes Develop individualized plans of care and provide counseling or referrals as needed Coordinate access to community resources, including financial assistance, housing, long-term planning, and mental health services Document services provided using the agency’s EMR system (WellSky/Kinnser) Communicate and collaborate with the physician, nurses, therapists, and other healthcare team members Educate patients and families about advance directives, end-of-life care, and long-term support options Maintain compliance with all applicable federal, state, and agency standards

Qualifications:

Master’s degree in Social Work from an accredited school Active Florida license as a Licensed Clinical Social Worker (LCSW) or Registered MSW (if supervision is provided) CPR certification Reliable transportation and valid driver’s license Professional liability insurance (or willingness to obtain) Home health or healthcare experience preferred Strong communication, documentation, and advocacy skills Knowledge of Medicare Conditions of Participation and community resources
