Home Health Aide Phoenix Rising Solutions Inc Callahan, FL 32011 Job Overview As a Caregiver, you will play a vital role in enhancing the quality of life for individuals in need of support. You will work closely with individuals who may have disabilities, memory care needs, or require home health assistance. Responsibilities Provide direct support to clients by assisting with daily living activities such as bathing, dressing, and grooming. Monitor patients’ health by observing their physical and emotional well-being, reporting any changes to the appropriate healthcare professionals. Assist clients with mobility needs, including heavy lifting and transferring them safely from one location to another. Maintain a clean and safe environment for clients by performing light housekeeping tasks as needed. Engage clients in meaningful activities that promote social interaction and cognitive stimulation. Support individuals with autism or other disabilities by implementing personalized care plans tailored to their unique needs. Document patient observations and care provided accurately to ensure continuity of care. Skills Strong patient monitoring skills to effectively observe and report changes in clients’ conditions. Experience providing direct support to individuals with disabilities or special needs. May require ability to perform heavy lifting and assist clients with mobility challenges safely. Knowledge of memory care techniques to support clients experiencing cognitive decline. Proficient in maintaining cleanliness and hygiene standards within the home environment. Compassionate demeanor with excellent interpersonal skills to build trusting relationships with clients and their families.
