• Job Summary
• The Home Health Aid is responsible for monitoring the health status of persons with disabilities or illnesses and addressing their specific health-related needs.

Work is conducted within the client’s residence following the established care plan and designated responsibilities. Additionally, assistance will be provided with routine healthcare tasks and activities of daily living, including feeding, bathing, toileting, and ambulation.

• Essential Duties and Responsibilities
• The essential functions of this position encompass, but are not limited to, the following:
• Maintain comprehensive records of.. clients care, condition, progress, and any issues, and communicate observations with the supervisor or nurse manager.
• Assist Clients with ADLS and mobility, including transitions in and out of beds, baths, wheelchairs, or automobiles. Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert.
• Offer emotional support and instruction to patients and their families in needed areas.
