Benchmarkis looking for a Licensed Clinician – Licensed Clinical SocialWorker (LCSW) or Licensed Professional Counselor (LPC) or LicensedMaster Social Worker (LMSW) or Licensed Associate ProfessionalCounselor (LAPC) to join our team in our crisis support homes foradults with IDD in Savannah and Bloomingdale, GA. RESPONSIBILITIES

Provide support toclients and staff working in the Benchmark supports and crisishome. Coordinate the short-term out-of-home placements withstakeholders. Develop discharge plan detailing the discontinuationfrom crisis support services. Follow up with individual’s IDT toensure that they adhere to crisis-service-recommendedsupport/services. Assist in providing information to the mobilecrisis team. Facilitate referrals quickly to prevent escalation ofcrisis. Establish and maintain record keeping functions;integration with CIS system. Complete necessary documentation tofacilitate reimbursement through primary funding sources. Developand implement safety plans as appropriate. Report any suspectedabuse, neglect or exploitation to supervisor or department head.

Must be willing to flex schedule accordingly to provide services,supports, care and treatment to individuals as required.

QUALIFICATIONS
