We work with people at home,at work, and in the community providing crisis response. Benchmarkis looking for a Licensed Clinical Social Worker (LCSW) or LicensedProfessional Counselor (LPC) or Licensed Marriage Family Therapist(LMFT) to join our team in the crisis support homes for adults withIDD in Acworth, GA. The Clinician will provide case managementservices to individuals who are receiving services in the program.

RESPONSIBILITIES
Provide all aspects ofcase management including, but not limited to, facilitatingreferrals, coordinating intake and discharge, tfollow-ups withstakeholders, etc. Provide support to clients and staff working inthe Benchmark supports and crisis home. Coordinate the short-termout-of-home placements with stakeholders. Develop discharge plandetailing the discontinuation from crisis support services. Followup with individual’s IDT to ensure that they adhere tocrisis-service-recommended support/services. Assist in providinginformation to the mobile crisis team. Facilitate referrals quicklyto prevent escalation of crisis. Establish and maintain recordkeeping functions; integration with CIS system. Complete necessarydocumentation to facilitate reimbursement through primary fundingsources. Develop and implement safety plans as appropriate. Reportany suspected abuse, neglect or exploitation to supervisor ordepartment head. Must be willing to flex schedule accordingly toprovide services, supports, care and treatment to individuals asrequired.
QUALIFICATIONS
