Job Description

GENERAL SUMMARY

The Community-Based Direct Support Professional (DSP) provides person-centered instruction, support, and serves as a positive role model for individuals with intellectual and developmental disabilities participating in the Community Based Day Support (CBDS) program. The DSP supports participants in areas such as pre-vocational/job readiness, socialization, community integration, education, and personal wellness.

ESSENTIAL JOB FUNCTIONS

• Ø Promote and uphold the dignity, rights, and personal liberties of individuals served. Ø Establish and maintain positive, supportive relationships with individuals in the program. Ø Assist in the planning and delivery of individualized services, including independent living skills, community self-management, social-emotional development, and behavioral support. Ø Support individuals in expressing preferences and making choices based on person-centered assessments and service plans. Ø Develop goals and objectives using the Individual Service Plan (ISP) format and oversee the implementation of relevant activities. Ø Facilitate opportunities for meaningful community engagement and inclusion. Ø Provide physical support as needed, which may include assistance with personal care. Ø Ensure compliance with all agency policies and procedures, as well as regulations from funding sources. Ø Communicate regularly with management regarding planned activities, progress, and any emerging needs or concerns. Ø Maintain professional relationships with individuals, coworkers, supervisors, community members, and external partners. Ø Attend and actively participate in staff meetings, required trainings, and professional development opportunities. Ø Serve as a mandated reporter to the Disabled Persons Protection Commission (DPPC) in cases of suspected abuse or neglect.

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

Ø 3 years of experience working with adults with disabilities preferred. Ø Demonstrated ability to teach and model vocational or life skills. Ø Must possess a valid Massachusetts driver’s license. Ø Completion of all required trainings in accordance with state and agency guidelines upon hire.

SUPERVISORY RESPONSIBILITY

Ø No supervisory responsibility.

WORKING CONDITIONS/PHYSICAL DEMANDS

Ø Frequent walking, standing, bending, and mobility within the community. Ø Ability to lift a minimum of 25 pounds.
