In this role, you will provide essential support to individuals in their homes, assisting them with daily living activities and ensuring their comfort and safety. Your contributions will play a vital role in enhancing the quality of life for our clients, allowing them to maintain their independence while receiving the care they need. Responsibilities Assist clients with daily living activities such as bathing, dressing, grooming, and meal preparation. Monitor and record vital signs, including temperature, pulse, and blood pressure as required. Help clients with mobility and transfers, ensuring safety while lifting or repositioning them. Maintain accurate medical records and documentation of care provided in compliance with HIPAA regulations. Provide companionship and emotional support to clients, fostering a caring environment. Assist with medication reminders and ensure adherence to prescribed health plans. Collaborate with healthcare professionals to implement individualized care plans tailored to each client’s needs. Participate in training sessions to stay updated on best practices in home community care. Experience Previous experience in long-term care or home health settings is preferred but not required. Familiarity with medical records systems and documentation practices is advantageous. Ability to perform basic health assessments and understand the importance of vital signs monitoring. Strong interpersonal skills with a genuine desire to assist others in a caring manner. Knowledge of HIPAA regulations and commitment to maintaining client confidentiality.
