Per Diem Requirements:
Completed a Certified Nursing Assistant (CNA) training program and have an active license Active CPR Certification 1 year of relevant experience as a CNA Compassion, integrity, and a commitment to providing excellent care The ability to handle situations with confidentiality and maturity The ability to read, write, and speak English Commitment and dependability Rotating weekend and holiday on-call responsibilities
Primary Job Responsibilities:
Care for patients in their homes in conjunction with the rest of the clinical team. Follow a personalized care plan for each patient that is assigned by the case manager who is available to support you in your responsibilities. Assist with activities of daily living (ADLs), light meal preparation, companionship, respite when applicable. Promote a safe environment and assist with ambulation/transfers. Maintain communication with the interdisciplinary team and the patient’s caregivers.
Pay:
$22.00 – $24.00 per hour
