Job Description

Direct Support Professionals Direct Support Professionals facilitate theISP goals of adults with disabilities and other psychiatric conditions,and work with their teams to provide an atmosphere of positive regardthat promotes day-to-day choices, greater independence, and theopportunity to participate in and contribute to community life. Stafflearn, practice and model approaches of engaging individuals withmethods associated with Positive Behavioral Supports. Direct SupportProfessionals report directly to the Program Manager unless otherwiseinstructed. Job Responsibilities 1. Individual Personal Support Provideassistance to individuals receiving services & ensure needs are beingmet. Assist all individuals as needed/required with all activities ofdaily living in keeping with PBS training modules. Assist allindividuals with personal hygiene, including toileting, shaving,bathing, brushing teeth.

which may includedressing, eating, communicating and mobility

Housekeeping:

laundry,cleaning bathrooms, bedrooms, and common areas. Planning healthy meals,grocery shopping, meal preparation, and clean-up Personal finance: whichmay include handling cash, accounting for purchases through receipts anduse of tax exempt card as directed by the Program Manager. 2. ProgramSupport Maintain a working knowledge of both the individual’s goals andobjectives as outlined in the ISP/IAP/ IEP and PBS principles foraccomplishing these goals. Assist individuals by respectingindividual’s personal beliefs, choices and interests. Remainknowledgeable of each individual’s level of supervision requirements.

Follow and implement behavioral treatment plans as required. Performproper restraints as necessary/indicated. Provide input & feedback tomanagers/clinicians regarding programs & plans, including PBSprinciples. Complete all required paperwork by the end of scheduledshift: (i.e. data, logs, clinical notes etc) Create opportunities thatprovide social and leisure activities of interest to the personreceiving supports. Administration, documentation and ordering ofmedications in accordance with MAP regulations and policy Assistindividuals with medical care as directed by the Program Manager 3.Operational Support Ensure a preventative maintenance approach toprogram upkeep and promptly report maintenance problems to the ProgramManager/Facilities Department.

Preventative maintenance:

yard clean up,snow shoveling, clearing of egresses Ensure the individuals supportedhealth and safety by maintaining a clean, well-kept environment. DrivesCompany and/or personal vehicle to transport individuals to theiractivities. 4. Performance Expectations Act in a professional manner andcommunicate effectively with others, maintain a positive attitude ofcompassion, cooperation, and courtesy. Employ PBS principles wheninteracting with individuals. Treat all people with dignity and respect,and give positive, unconditional regard. Report for work on time, asscheduled and work shift in its entirety. Maintain confidentiality inall aspects of the job. Communicate concerns to Supervisor. Maintainprofessional boundaries between employees, individuals, and family.

Qualifications Must be at least18 years of age and have a GED or High School diploma Must have anoperational and insured vehicle for use during work and a validdriver’s license. Experience working with individuals with disabilitiesis preferred.
