  • Position Overview:
  • You will be directly working with patients, following a one-on-one care plan in patient homes.

You will provide essential patient care, observing and reporting on patient conditions, assisting with activities of daily living, and ensuring a safe and supportive environment for our clients. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You will collaborate closely with our nurses to deliver comprehensive care tailored to each individual’s needs. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

  • Responsibilities:
  • Perform patient observation and provide direct care as required.
  • Assist with activities of daily living and personal hygiene.
  • Support patients with mobility, including lifting and transferring.
  • Collaborate with healthcare providers to implement care plans.
  • Provide emotional support and companionship to patients.
  • Assist with household tasks such as light cleaning and laundering.
  • Ensure adherence to infection control protocols.
  • Give medication reminders.
  • Engage patients in exercises or other activities.
  • Maintain records of patient care, condition, progress or problems to report and discuss with supervisor.
  • Qualifications:
  • Graduated from an accredited Home Health Aide program.
  • High school diploma or GED.
  • Prior experience in home care, hospice care, or related healthcare settings is preferred.
  • Proficient in patient care practices and techniques and long-term care principles.
  • Experience working with individuals requiring hospice or dementia care is advantageous.
  • Ability to operate medical equipment such as lifts or other assistive devices.
  • Driver’s license required.
  • CPR certification preferred.
  • Comfortable working in home environments with pets.
  • Capability to lift up to 25 pounds safely.
