JOB DESCRIPTION SUMMARY

The House Lead is responsible for day-to-day operations of the residential program. This role is also responsible for the overall well-being of program participants by cultivating an environment that fosters independence within a safe learning environment and that encourages the enhancement of positive relationships.

ESSENTIAL FUNCTIONS

Assist in developing, implementing and monitoring Individual Program Plans (IPP). Advocate for individual(s), i.e., attend all meetings, ensure all medical, dental and therapy appointments are scheduled and kept and assist in developing and maintaining family and community relationships. Maintain accurate records including daily logs, individual files, and accident and incident reports. Attend to individuals’ personal care needs. (This may include assisting individuals in and out of wheelchairs, use of Hoyer lift and maintaining good hygiene.) Provide quality care to program participants in an environment that promotes their rights, dignity, freedom of choice and individuality. Support daily interactions between participants and staff to promote a culture of positivity, respect, commitment and optimism. Act as an exemplary role model to program staff by demonstrating great attendance, accountability and strong work ethic. Train new staff in all areas including individual plans and personal care requirements. Assist with scheduling staff to ensure adequate coverage. Fully understands all aspects of participants’ rights and is responsible for promptly reporting any evidence of abuse or violation of participants’ rights. Communicates and interacts effectively and tactfully with all Participants, CW team members, family members, guardians, case workers, medical providers for Participants, and other stakeholders. Complete all necessary reports and submit (when appropriate) to Business Office in a timely manner. Must maintain medications and ensure adequate supply at all times. Transport individuals, as needed, using personal vehicle (i.e., to appointments, work, etc.)

OTHER RESPONSIBILITIES

Be sure any location where individuals use CW’s services, (i.e., residences, day services or office locations) are kept clean and safe. Maintain open and focused communication with team members regarding goals and progress of program participants. Fully understands and abides by the agency’s code of ethics and all other CW policies and procedures.

Project professional image at all times. Be a role model for other Direct Support Professionals and all team members relative to coaching techniques, positive behavioral support strategies, professionalism and interaction with consumers. Troubleshoot and communicate concerns to appropriate support person. Be proficient in seeking out community resources and supports for consumers. Ensure daily operations adhere to CARF accreditation standards, as well as State and Federal regulations. Attend staff meetings and assist in communicating concerns from program participants. Be proficient in documentation and assist in providing oversight of the program records. Provide crisis interventions as needed. Other duties as specified.

PHYSICAL WORK TOLERANCES

This position is very active and requires frequent standing, walking, bending, kneeling, stooping, and crouching. Must frequently assist in lifting non-living loads up to 50 lbs. Must frequently assists participants’ with transfers. While performing this job the employee is regularly required to talk, hear, and see to the extent necessary to perform the essential functions of the position.

QUALIFICATIONS

High School Diploma or GED. Direct Support experience or previous experience in related field preferred. Valid U.S. Driver’s License Must have satisfactory driving record per agency’s insurance company standards. Must have ability to communicate well in the program, including the ability to read, write, speak and understand the English language so that the support of the participant receiving services can be met, and so that the relationship and work with co-workers, doctors, therapists and others in the community is successful. Must be able to perform basic math skills to the extent necessary to perform the job. Must have adequate writing skills to maintain documentation standards, reports, and any other writing expectation in the program. Must have or be willing to obtain trainings within the Agency’s specified timeline including but not limited to: DSP, CPR/FA, CRMA, Welle, CLF.
