Provide individual and family counseling in the home setting
Assist with establishing advanced directives and durable power of attorney
Facilitate the use of community and home care resources
Attend and actively participate in interdisciplinary group (IDG) meetings
Make referrals to long-term care facilities as needed
Develop and facilitate support groups
Collaborate with fellow Social Workers and the Hospice team
Build and maintain professional relationships with staff and referral sources What We’re Looking For
Master’s degree in Social Work (MSW)
Home Health experience preferred
Ability to work independently and collaboratively
Professional demeanor and appearance
Excellent attendance, punctuality, and time management skills
Strong problem-solving, communication, and multi-tasking abilities
