Responsibilities:

What are the care requirements of this client? Meal Preparation Light Housekeeping Engagement Bathing/dressing Standby assistance

Toileting Medication Reminders Notes:

Client will need transportation to appointments/errands/shopping, caregiver car will need to be used. Client has Dementia. Needs reminders for bathing/dressing. Some assistance with toileting.

Requirements:

What is required from you? Must be at least 18 years of age or older Must be able to pass a background check Must be authorized to work in the United States Must have a valid ID or Driver’s License Must be reliable
