- Provides support for individuals with disabilities in the community.
Assists the person receiving services in identifying and communicating his or her requests and needs for services and supports. Implements services and supports to address the persons goals and desired outcomes as identified through each persons Centered Plan. Seek to understand the individuals in the context of their personal history, their social and family networks, and their hopes and dreams for the future. Relies on direction and supervision to accomplish the job.

DUTIES AND RESPONSIBILITIES
- Provide education on human, legal, and civil rights.
- Promote healthy living and prevent illness and accidents.
- Increase awareness of self-advocacy and support individual choice and decision-making.
- Accompany people to work, the movies, concerts, shopping, and other community outings.
- Teach independent living skills.
- Provide transportation to appointments.
- Foster positive relationships within the community.
- Promote decision-making about living, work, and social relationships.
- Assist with personal care and bathing, help around the home (laundry, cleaning, and decorating)
- Document the care you provide.
- Understand diverse challenges of those living with disabilities and use effective advocacy strategies.
- Connect people with resources to further explore opportunities in their career, education, volunteerism, etc.
- Promote effective communication.
- Assist in medication administration where state law allows.
- Protect confidentiality.

REQUIREMENTS
- 18 Years and older
- Valid Social Security Card
- Valid Picture ID
- Auto Insurance with your name as the insured driver
- High school diploma/ GED (Recognized by the United States department of education)
- Tuberculosis Written test (Proof of skin test within the last 10 years)

- CPR and First Aid (Offered @ HRU at applicants’ expense $50.00)
- (Must be American Red Cross / American Heart Association)
- CPI Training (provided)
- Universal Precautions(provided)
- Back Safety(provided)
- DSP Training Core Competencies Training (Provided)
- Documentation Systems Training (Provided)
- Ongoing Annual Training
