POSITION QUALIFICATIONS

SUMMARY OF JOB FUNCTIONS
Provide exceptional care in the home and/or community to support needs such as safety skills, personal care, communication skills, etc. Ensure client’s safety and security by supervising in the home environment. Assist in developing daily living skills by identifying and meeting social, environmental, and health care needs. Perform and/or teach home management tasks, basic meal preparation, light housekeeping, home safety, handling emergencies in the home, and infection control with excellence. Maintain comprehensive written documentation tracking daily goals and progress monitoring. Follow the client’s Plan of Care, while ensuring service is delivered in a compassionate, caring, and respectful manner and in accordance with relevant Agency policies and industry standards. - Travel to clients’ place of residence using a reliable means of transportation.
- Submit required documentation in a timely manner (i.e. records of daily activities, observations, and direct hours of service)
- Demonstrate ability to perform required client care tasks or activities or undergo training
- Attend orientation, in-service training sessions and staff meetings.
- In addition to scheduled hours, occasionally serving as backup caregiver/on call, as needed.
