Job Description

Direct Support Professional – Home and Community 3.3 3.3 out of 5 stars 121 Railroad St, Keene, NH 03431 Many flexible part-time schedules are available for Keene area and surrounding towns $750.00 Sign-on bonus Summary Home and Community Direct Support Professionals (DSP) are responsible for a variety of duties related to the overall care of an individual with a developmental disability or an acquired brain injury in their home and community settings. This includes direct care. Some of the duties and responsibilities vary in intensity and frequency depending on the individual supported. Essential Functions Assist the individual to develop and maintain independent living skills in areas including, but not limited to; social skills, cooking and nutrition, communication, money management, community integration, household duties, safety, health and personal hygiene. Provides medication administration, if the individual is unable to administer their own medication. This is determined by the agency nurse. Assist individual to access and engage in social, community participation activities. Supports individual to achieve the goals outlined in their individual service agreement and behavior plan. Communicates with the Program Manager any concerns impacting the individual’s well-being including injury, illness or change in emotional and/or physical status. Provides transportation to appointments and community activities. Accurately documents all program data, including but not limited to: daily notes, medication logs, incident reports, progress notes, mileage reimbursement forms, seizure logs, weight, in a timely manner. Follows safety protocols to foster a safe working environment. Reports unsafe conditions, environment, and equipment to the Program Manager. Adheres to all relevant policies and procedures. Maintains individual’s confidentiality. Recognizes and acts on the legal responsibilities concerning the safety and welfare of the individual. Education/Experience Minimum high school diploma or equivalent. Ideal candidate has one to two years’ experience working with people with developmental disabilities. Required to complete all MDS assigned trainings with the first 6 months of employment and then on a annual basis.
