These positions are often found within organizations contracted by state Divisions of Developmental Disabilities (DDD), such as in New Jersey, Arizona, and other U.S. states

Key Responsibilities Support Coordination :

Develop and implement Individual Support Plans (ISPs) in collaboration with individuals, families, and service providers. Regularly monitor and adjust plans to meet evolving needs.

Community Integration :

Assist individuals in accessing community resources, including employment opportunities, recreational activities, and social services. faithcareus.com •

Crisis Intervention :

Documentation and Compliance :

Maintain accurate records of services provided, ensuring compliance with DDD regulations and timely submission for billing purposes. •

Educate individuals and families about available services and rights, advocating on their behalf to ensure access to necessary supports.

Typical Qualifications:

Experience :

At least one year of experience working with individuals with developmental disabilities or in a related social services role.

Skills :

Strong communication, organizational, and problem-solving skills.

Certifications :
