Responsibilities:

Assist clients with daily living activities: bathing, dressing, grooming, toileting, and mobility Provide companionship and emotional support Perform light housekeeping, meal preparation, and medication reminders Follow the individualized care plan provided by the nurse Report any changes in the client’s condition to the agency Maintain accurate and timely documentation

Requirements:
